Zapier is one of the few backend services which can be used by a pure frontend page, it doesn’t support CORS directly, it doesn’t complain too much.

Just create a simple form with LetsForm Designer like this one.

Then create a table on Zapier

Then attach a Zap automation to it

Select “Bring data in” and open the link below to edit the Zap

Edit the trigger and select “WebHook by Zapier” and, in the following screen, Event → Catch Hook, leave “Pick off child key” empty

In the following screen you can grab the webhook URL for this Zap
